OUR APPOINTMENT POLICY
We reserve time for each patient in our practice. An appointment written in our schedule with your name on it is a bond of trust that we will be here to serve you and you will be present for that appointment. Therefore, our office policy in this regard is extremely firm. You must be present for all scheduled appointments. A broken appointment charge is assessed for all cancellations made with less than 48 hours notice. We do not accept recorded messages as appointment cancellations or changes. Your signature will be required on your first visit as an indication that we have mutual respect for each other’s time.

PAYMENT POLICY
Payment for treatment is expected at the time the service is performed.

Payment Options
For your convenience, payment options include: cash, check, Discover, Mastercard, or Visa. Care Credit is also available (we pay your interest for the first 12 months). Requires advance notice and pre-approval.

Insurance Forms
As a courtesy to you, we will submit your insurance claim form for you on the date of service. The insurance company will mail your reimbursement directly to you. Any questions regarding the terms of your policy should be directed to your insurance carrier or your company’s human resources department. We will do everything we can for you to make full use of your insurance benefits.

Unpaid Balances and Returned Check
Unless prior arrangements have been made, balances older than 60 days will be subject to interest charges of 1.5% per month. Balances older than 90 days will be given to an attorney for collection. Return checks will have an additional fee of $40 added to your account.
We understand that temporary financial problems may affect timely payment of your balance. We encourage you to communicate any such problems to us so that we can assist you in the management of your account.
