We respect our patients, and our goal is to provide you with the highest quality care in an atmosphere of mutual trust.
- Our practice is truly a family practice that has thrived over the years based on word-of-mouth referrals. We often treat many members of the same family, and some of our best patients are referred by some of our best patients.

Medical and Dental History Questionnaire. We need this information so that we can treat you safely, and so that we know which procedures you have undergone in the past.
Statement of Privacy Practices. We realize that your medical information is both private and sensitive, and we strive to keep it that way.
Acknowledgement of Receipt of the statement of privacy practices. We want to know that you have read and reviewed it.
The Children’s Medical and Dental History Questionnaire, if you are bringing your child in for his or her first trip to the dentist.
